CBD (Cannabidiol) and Ketamine Therapy | Tovani Health
Epidiolex (Cannabidiol) (also: Charlotte's Web) — Non-psychoactive cannabinoid
Verdict at Tovani Health
Compatible; the CYP3A4 inhibition can slightly raise ketamine levels at high doses.
CBD and ketamine are compatible. CBD is non-psychoactive and does not produce the cannabis interactions covered separately. Two practical considerations: CBD inhibits CYP3A4 and CYP2C19 (the enzymes that metabolize ketamine), so at high doses it can modestly raise ketamine plasma levels; and CBD products are unregulated, so potency varies significantly between brands.
If you take Epidiolex regularly and are considering at-home ketamine therapy, the combination is safe with monitoring or dose adjustment. This page covers the brief pharmacologic context and what we do at intake.
How Epidiolex interacts with ketamine
CBD binds many targets weakly (CB1 negative allosteric modulator, 5-HT1A agonist, others). It inhibits CYP3A4 and CYP2C19 at clinically relevant doses, which can affect metabolism of other drugs.
What we do at intake
Disclose dose, brand, and route (oil, gummies, vape). High-dose Epidiolex (prescription, 10-20 mg/kg/day) has more interaction potential than typical OTC CBD doses (10-50 mg/day).
