My goal was to make it available without asking anyone to drive ninety minutes to an infusion clinic.”

What a first session actually looks like

The part nobody tells you.

Everyone’s worried about the same thing on their first dose: “Am I going to lose control in my own house?” Short answer: no. Here’s the long answer.

    You dose

    One tablet under the tongue. Support person in the room, phone nearby, lights low. You’re sitting or lying down somewhere you want to be for the next 90 minutes.

  2. T + 15 min

    It starts

    A gentle dissociation — the sense that your thoughts have more room. Most people describe it as the opposite of anxious. You can still hold a short conversation.

  3. T + 30–60 min

    The window

    This is where the antidepressant work happens. Eyes closed, music on, letting whatever comes up come up. You’re not unconscious; you’re introspective.

  4. T + 90 min

    Back

    Clear-headed within two hours. Most patients sleep well that night. No driving and no big decisions for the rest of the day.

    Eligibility

    Five-minute questionnaire

    A quick rundown of your history, medications, and what you’ve already tried. Takes about five minutes and tells me whether we should talk.

  2. 02

    Video visit with me

    30 minutes, same week

    I review your screening, ask about what you've tried, and if ketamine makes sense I'll send the prescription that same visit.

  3. 03

    Medication ships

    24–48 hours

    Sublingual tablets from a compounding pharmacy in Boca Raton or Ramsey. You pay the pharmacy directly, around $5 per tablet.

  4. 04

    At-home sessions

    Twice Weekly, then taper

    You dose at home with a sober adult nearby. I check in by video between sessions and adjust the dose if we need to.

FAQ

Questions I get asked a lot.

Is this real medicine, or is this a trend?

Ketamine has been FDA-approved as an anesthetic since 1970. Its use for depression is off-label but backed by 25+ years of trials. Spravato (the FDA-approved nasal spray) is the S-enantiomer only; what I prescribe is racemic ketamine — the full molecule, both S and R forms — taken sublingually, which is what most of the depression research actually used, at a fraction of the cost.

Why at home instead of an infusion clinic?

For maintenance and most first courses, sublingual works. Infusions are faster-acting but they cost $400–800 a session and require you to drive to a clinic. If you need IV, I'll tell you and help you find one.

Who shouldn’t take this?

Uncontrolled blood pressure, active psychosis, pregnancy, and active severe substance use disorder are the clear contraindications. I screen for all of them before prescribing.

Will I be addicted?

Sublingual ketamine at prescribed doses, on a tapering schedule, with physician supervision has very low abuse liability. I watch for it and I stop if I see it.

How fast will it work?

Most patients feel something after the first session. Durable response usually takes 3–6 sessions. If we've done 4 and you haven't felt anything, we stop — I'm not going to keep prescribing something that isn't helping.

Do I need a therapist too?

Strongly recommended. Ketamine opens a window; therapy is how you use what comes up. I can refer you to KAP-trained therapists in FL and NJ who work alongside this.

What does it cost, all-in?

$349 for the first month (includes the video visit and my supervision). Medication from the pharmacy runs about $5 per tablet. Most first courses land around $400–500 total for the month.

Why you and not one of the bigger platforms?

Because the bigger platforms aren’t practicing medicine — they’re running call centers with rotating providers. I’m the only doctor at Tovani. Same person reviewing your intake, same person prescribing, same person you message at week 3.
