Rifampin alongside ketamine requires individual evaluation. Rifampin is the prototypical strong CYP3A4 inducer in clinical use, similar in magnitude to carbamazepine. Patients on rifampin (typically for TB or specific Gram-positive infections) may experience meaningfully reduced ketamine exposure and blunted response. We do not refuse, but we plan honestly: response may be smaller during therapy, with normalization in the weeks after the course finishes.

How Rifadin interacts with ketamine

Rifampin strongly induces CYP3A4, CYP2B6, CYP2C9, and P-glycoprotein. Ketamine relies on CYP3A4 and CYP2B6 for clearance, so induction increases clearance and lowers plasma exposure for any given dose. Effect persists 1-2 weeks after the last rifampin dose.

What we do at intake

Disclose current or recent rifampin therapy. Tell us the indication and duration. We may delay starting KAP until at least 2 weeks after the course finishes for the most predictable response.
