Rifabutin alongside ketamine requires individual evaluation. Rifabutin is the preferred rifamycin in HIV patients on protease-inhibitor or integrase-inhibitor regimens (rifampin is too strong an inducer for those regimens). It's a meaningful CYP3A4 inducer β about 40% as potent as rifampin β so it can lower ketamine plasma exposure during therapy. Patients may need higher KAP doses to achieve equivalent effect, or response may be muted. We evaluate case-by-case rather than refuse.

How Mycobutin interacts with ketamine
Rifabutin inhibits mycobacterial RNA polymerase. Moderate CYP3A4 induction (intermediate between rifampin's strong induction and rifaximin's near-zero systemic effect). Effect persists 1-2 weeks after the last dose.
What we do at intake
Disclose dose, indication, and any concurrent HIV medications. We may adjust the KAP dose upward or delay starting until at least 2 weeks after the course finishes.
