Nitroglycerin and Ketamine Therapy | Tovani Health

Nitrostat (Nitroglycerin) (also: Nitro-Bid, Nitro-Dur)Organic nitrate (vasodilator)

Verdict at Tovani Health

Compatible; the absolute PDE5-inhibitor contraindication matters more than the KAP interaction.

Nitroglycerin and ketamine are compatible. The cardiovascular interaction with KAP itself is minimal. Far more important: nitrates are absolutely contraindicated with PDE5 inhibitors (sildenafil, tadalafil, vardenafil) because of the risk of severe hypotension. That contraindication is independent of ketamine but worth flagging since erectile dysfunction medications are common in the cardiac patient population.

If you take Nitrostat regularly and are considering at-home ketamine therapy, the combination is safe with monitoring or dose adjustment. This page covers the brief pharmacologic context and what we do at intake.

How Nitrostat interacts with ketamine

Nitroglycerin releases nitric oxide, producing rapid vasodilation. Used for acute angina (sublingual) or chronic ischemic heart disease (transdermal or oral isosorbide preparations). No direct ketamine PK interaction.

What we do at intake

Disclose use (sublingual PRN for angina vs scheduled isosorbide). Do not take any PDE5 inhibitor (Viagra, Cialis) without your cardiologist's awareness.

Far more important: nitrates are absolutely contraindicated with PDE5 inhibitors (sildenafil, tadalafil, vardenafil) because of the risk of severe hypotension.
